Federal Contractors to Transition to New Form Immediately
The Office of Management and Budget (OMB) has renewed the Voluntary Self-Identification of Disability form, which was set to expire on January 31, 2017. While no changes have been made to the form, its use has been extended for three years.
Effective immediately, affirmative action employers must either download the renewed form(s) or update their electronic version(s) of the form to reflect the new expiration date.
As a reminder, the disability self- identification form must be offered to applicants and new hires, as well as used to resurvey the workforce at least once every five years. The form is OMB-controlled and cannot be modified by employers . Completing the form is voluntary for applicants and employees.
Employers using an online application system should work directly with their applicant tracking system vendor to ensure the new form is displayed for applicants.
